Job Description

The Wake County Health and Human Services Senior & Adult Services Program offers a variety of home and community-based programs and services to seniors, adults with disabilities, and their families. The Senior & Adult Services Program consists of three (3) teams: Adult & Community Services, Adult Guardianship, and Adult Protective Services. Our goal is to maximize self-sufficiency, safety, health and independence so that seniors and adults with disabilities can remain safely in the community for as long as possible.  Additionally, we strive to maintain the highest quality of life for those who reside in adult and family care homes in Wake County. The Senior & Adult Services Program provides: (1) placement services, (2) case management services for people receiving Special Assistance/In-Home funds, (3) monitoring of adult care homes and day care programs, and (4) protective services to disabled adults who are alleged to been abused/neglected/exploited and in need of protection.   Additional services are provided to clients who have been declared mentally incompetent and who have no one else to assume decision-making responsibility. We are currently seeking an Adult & Community Services Social Worker to join this dynamic team! The Adult & Community Services Social Worker will monitor adult care homes and adult day care/day health programs for compliance with state licensure and certification rules. Responsibilities include:
  • Developing and implementing trainings in the community on issues pertinent to elderly and disabled adults
  • Providing case management services and administer funding for health and safety needs to disabled and Private Living Medicaid eligible consumers in the community at risk of adult care home placement
  • Providing case management services to elderly and/or disabled consumers over the Medicaid income limit in the community with brittle support systems needing support in getting their basic needs met
  • Evaluating and providing consultation and technical assistance to adult care home licensee and administrators for initial licensure and compliance with licensure rules
  • Frequently working in the capacity of liaison between consumers and residents of adult care homes

The Basics (Required Education and Experience)

  • Bachelor's degree in Social Work or related field
  • Two years of job-related experience
  • Valid Driver's License and safe driving record

Beyond the Basics (Preferred Education and Experience)

 
  • Experience working with elderly, disabled and adults with mental illness, intellectual developmental disabilities and substance abuse in licensed adult care homes, adult day care facilities and in the community
  • Experience working in fast paced environment responding to phone calls from the general public
