Duties and Responsibilities: This position is responsible for providing multiple, complex services to elderly and disabled adults including transportation for medical, dental, and therapeutic services; training in household management; nutrition, community skills and safety measures in the home. This position may also at times provide support to the Children’s Services Unit to provide multiple mandated services to children in the agency’s custody to include transportation for medical, dental and therapeutic services, supervision of family visitation, reporting of visit observations to the case managers, testifying in court as requested, and keeping updated case records as required by case standards. This position may also occasionally be required to assist families to find housing, daycare, and transportation.
Minimum Education and Experience Requirements: Demonstrated possession of knowledges, skills, and abilities gained through two years of experience in performing similar instructional and community placement tasks; or graduation from a two-year associate degree program designed to prepare support personnel for human services agencies which includes a six-month practice/internship in a similar setting. One year of the above experience may be substituted for the practice.
