Job Description

The Forsyth County Department of Social Services is seeking to hire a Sr. Social Worker for its Adult Home Specialist Program.
The social worker in this position will assist individuals and their family members with identifying appropriate placement in a licensed facility within the community and provide assistance to individuals that are in a facility but are choosing to move back into the community. The preferred candidate in this position will be assigned to monitor and investigate complaints for facilities and Adult Day Care Centers and facilitate initial licensing when there is a change of ownership, new license, or recertification.  Additional job responsibilities /duties inlcude: 1) Required completion of all required paperwork per state regulations including but not limited to Monitoring Reports, Corrective Action Reports, and Complaint Investigative Reports and serves with the state on survey teams for annual licensing surveys and prepares report to be included in state document, when necessary. 2) Maintains documentation regarding compliance and follow up for licensed facilities including Type A and B penalty recommendations per state statutes and follow up on any concerns that were discovered during a monitoring visit. 3) Assists with relocation of residents from facilities that are being closed under state orders and provides emergency assessment, placement and case management until the issue is resolved and residents are in a safe environment. This position offers a unique opportunity for social workers to work with vulnerable adults and assist facilities and staff in understanding and meeting state licensure standards and provides assistance and information to persons in the community that are seeking an alternative setting i.e. family care home, assisted living, nursing home etc. Additionally this position rotates the duties of Adult Services Intake with other social workers on a regular basis. This position requires excellent assessment skills, sound judgment, and the ability to serve adult clients and families from all socioeconomic backgrounds with a range of mental capacities without personal bias.  Excellent written and verbal communication skills are also necessary. Primary work hours are Monday - Friday, 8:00 a.m. - 5:00 p.m. with some flexibility in scheduling to meet the needs of the families served within mandated timeframes. Master's degree in social work from an accredited school of social work or; Bachelor's degree in social work from an accredited school of social work and completion of the Child Welfare Collaborative or; Bachelor's degree in social work from an accredited school of social work and one year directly related experience or; Master's degree in counseling in a human services field and one year of social work or counseling or; Bachelor's degree in a human services field from an accredited college or university and two years directly related experience or; Bachelor's degree from an accredited college or university and three years of directly related experience. Valid driver's license required. Bilingual candidates (English/Spanish) preferred.
