Job Description
The Pregnancy Care Manager provides case management for a designated caseload of pregnant women, with sometimes-complicated pregnancies or critical environmental concerns. The Pregnancy Care Manager collaborates with the Client, and other Community and Health Care Providers to facilitate client access to those health and social services which enhance the patient’s health/wellbeing and her ability to adhere to the prescribed medical treatment regimen in the most efficient and cost effective manner possible. The Pregnancy Care Manager provides services to the patient in a variety of Community and Clinical settings.
This is a Supervisor level professional social work position in providing primary, preventive and rehabilitative care management services to individuals and families in the Public Health Programs. Employees independently demonstrate and implement techniques of counseling clients on the promotion of health and prevention of disease through outreach. Employees will be expected to assist in the data collection and review in any project they are associated with. Employees may manage a program-level support activity; participate in program or agency-wide planning and quality assurance activities; contribute observations or data related to patient or community trends or practices ideas for improved program performance; participate in record audits and other program reviews.
This position provides direct oversight of two Care Management programs: Pregnancy Care Management and Care Coordination for Children. This position oversees schedules, time records, leave requests for employee time and participates in performance evaluations and other quality assurance activities.
This position is also responsible for program development, setting program policy and establishing program goals. This position assesses program guideline adherence, program performance, and measures program adherence with program objectives and meeting program outcomes and benchmarks. Work is performed under the direct supervision of the Public Health Nursing Director I, and is evaluated by review of records, reports, in conferences, approval of seminars and observation.
Graduation from a four-year college or university with a degree in human services, social work, or related field and three (3) years of experience in social work and human services; or an equivalent combination of education and experience. Master's degree in social work from an appropriately accredited institution and one year of directly related experience is preferred.
