Job Description

Primary Purpose Under the general supervision of the Senior Center Coordinator the primary purpose of this position is to assist in the performance of administrative work in the overall operation of a Home Delivered Meals program in accordance with the Federal and State rules and regulations governing nutrition programs for the elderly. Employee is responsible for initial assessments and biannual reassessments that includes: gathering social history information of the client, determining eligibility for clients in the Home Delivered Meals programs, answering questions as well as giving explanations and interpretations of policy for the Home Delivered meals program, making referrals to other services, maintaining contact with client and family to ensure needs are met. Employee is responsible for making home visits and telephone contact with clients and responsible parties, ordering meals and monitoring meal delivery, checking meal temperatures, and making sure that a full meal is available for each participant. Works directly with the caterer regarding meal quality, adjustments to menu and reconciliation of billing. Position schedules volunteers and part time driver for meal delivery. Also delivers routes if there is a shortage of drivers.  Work includes record keeping pertaining to clients, and volunteers. Employee is responsible for assisting in the recruitment, retention, and supervising of volunteers. Essential Functions 95%
  • Provides daily oversight to home delivered meals program clients and volunteers to ensure nutritionally balanced meals to the homebound elderly in accordance with Federal and State rules and regulations governing home delivered meals programs for the elderly.
 
  • Recruits and conducts initial and ongoing training for volunteers.  Coordinates route schedules daily for volunteers..  The number of volunteers this position works with is approximately 75.
 
  • Gathers information from clients, relatives and other contacts for confidential assessments with new referrals and biannual reassessments for home delivered meals. Completes program forms, including client registration and supplemental forms.  Staffs with Senior Center Coordinator for new referrals,  changes in participant’s condition, status, or living environment, and 6 month re-assessments for eligibility determinations of each participant.
 
  • Develops daily delivery sheets and implements route changes daily as needed, notifies volunteer drivers to ensure availability and maintains a monthly delivery schedule. Works with county part time driver to ensure all routes are covered and delivered timely. Will provide delivery of meals if there is a shortage of drivers.
 
  • Reviews information and concerns presented to worker by drivers and make follow up contact as appropriate which could include a call to the participant or responsible party, a home visit and or referral for other services.
 
  • Completes program forms, including client registration forms, caterer’s assessment, daily count forms, weekly summaries, and program surveys.
 
  • Public contact with participants, responsible parties, volunteers, community service partners and agencies. Maintains contact with clients, relatives and other contacts as required by standards to give information, answer factual questions, give explanations and interpretations of agency/program policies and procedures, and make referrals.
 
  • Orders and receives meals, checks meal temperatures, prepares meal carriers for volunteer use. Makes sure meal carriers are clean for the next day delivery.
 
  •  Establishes a file for each participant, collects attendance information as well as information on notable health events and potential changes in eligibility, living environment, well-being, and cognitive function.
 
  • Provides nutrition education to clients twice a year as required by standards.
 
  • Assigned to provide supervision, instruction and assistance with activities provided in the Group Respite Program. This program provides a caregiver respite service for individuals with Alzheimer’s or related conditions.
 
  • Greets and interacts with participants and volunteers through conversation and scheduled activities.
 
  • Collects program income and turns said funds in for deposit daily.
 
  • Provides coverage for Congregate and Senior Center programming.
 
  • Maintains records of participation, prepares periodic reports on the program
 
  • Assists with recruitment, training and supervision of volunteers.
 
  • Assists with fund raising activities.
  5% The position adheres to other duties assigned by the Human Services Coordinator III, Program Manager, and/or Director at their discretion.  The position also assists in disaster relief efforts that includes but is not limited to shelter duty. Assignment to emergency shelter duty during times of potentially dangerous or manmade disasters is an essential function of this position and you shall be required to fulfill these duties when instructed.   Minimum Education and Experience Bachelor's degree in a human services field from an appropriately accredited institution; Bachelor's degree from an appropriately accredited institution and one year of directly related experience; or an equivalent combination of education and experience.
