Social Worker II – Adult Services

This position serves as Case Manager for persons receiving services through the Community Alternatives Program (CAP) Guardianship and Representative Payee Services.  A worker in this position acts as the Guardianship / designee for persons adjudicated incompetent on behalf of the DSS Director. This unit administers the following programs and services:  Adult Protective Service (APS) and Outreach, Guardianship, Licensure and Monitoring of Family Care and Adult Care Homes, Adult Day Care Monitoring, Adult Day Case Management, Community Alternatives Program (CAP), In-Home Aide, the placement of clients in facilities, Representative Payee Services, Special Assistance In-Home Services, and Medicaid Administrative Claiming.
