Job Description
This position provides case management to adults at risk of institutionalization. Position is responsible for receiving requests for services, assessing need(s) and determining eligibility. The worker locates, coordinate, and monitor the delivery of needed medical and mental health services. Receive reports of maltreatment of vulnerable adults, evaluate their need for protection and coordinate services. This position is also, responsible for On-Call duty for after hours, weekends, and holidays. Disaster Duty is an essential function of this position. Possess considerable knowledge of social work principles, techniques, practices, and their application to complex casework and community problems. Possess considerable knowledgeable of a wide range of medical, behavioral, psychosocial problems and treatments. The ability to establish and maintain effective working relationships and members of case load and their families, as well as members of the community and professional partners. |
Knowledge, Skills and Abilities
Minimum Training and Experience -
Master's degree in social work from an accredited school of social work; Bachelor's degree in social work from an accredited school of social work and completion of the Child Welfare Collaborative; Bachelor's degree in social work from an accredited school of social work and one year directly related experience; Master's degree in a human services field and one year of directly related experience; Bachelor's degree in a human services field from an accredited college or university and two years directly related experience; Bachelor's degree from an accredited college or university and three years of directly related experience. |
