Job Description Permanency Planning (Foster Care) Social Worker A Permanency Planning Social Worker is assigned to work with families after a petition is filed and the child(ren) have been removed from the home of the parent/caregiver and placed in the custody of the Department of Social Services.  The primary goal of the position is to address the safety concerns and risk factors that led to the need for removal and reunify with the parent/caregiver.  If reunification cannot be achieved, the social worker must identify and work towards an alternative plan such as adoption, guardianship, or custody with a relative or court approved caregiver. The ultimate goal is permanency and safety for the child.  The Social Worker must connect the parents to appropriate resources, ensure attendance and completion of the services and to promote positive change. Concurrently, the Social Worker must also address children’s well-being needs and support the placement provider to help maintain the stability of the placement while working towards permanence. This position necessitates working as a team in collaboration with the court system, the Guardian Ad Litem, community partners and placement providers and reporting progress or lack thereof to the court.   Please note this is a fast-paced, multi-faceted long-term intervention and requires the Social Worker to have crucial conversations with families about uncomfortable topics. There will be a significant amount of time spent in family homes and in the community. Job responsibilities include:
  • Create a case plan with parents/caregivers to address the needs identified in the petition
  • Make regular home visits/community visits with parents and child(ren)
  • Supervise visits with parents/caregivers and transport child(ren) at times
  • Conduct private discussions to assess child safety and risk on an ongoing basis
  • Connect families to community resources to address safety and risk factors
  • Complete timely, concise, detailed documentation
  • Complete court reports and testify at court hearings
  • Collaborate with team members to include service providers to make informed decisions regarding the permanent plan for child(ren)

Qualifications Master’s degree from an accredited school of social work and one year of social work experience; or a Bachelor’s degree from an accredited school of social work and two years of social work or counseling experience; or Master’s degree in a counseling field and two years of social work or counseling experience; or a four-year degree in a human services field or related curriculum and three years of social work or counseling experience; or graduation from a four-year college or university and four years of experience in rehabilitation counseling, pastoral counseling or a related human service field providing experience in the techniques of casework, group work or community organization; or an equivalent combination of education, training and experience that provides the required knowledge, skills and abilities.  One year of work experience can be credited for completion of the social work collaborative. **Please submit a copy of college transcripts with application** Physical Requirements Must be physically able to operate a variety of automated office machines, which include typewriters, calculators, copiers, facsimile machines, etc.  Must be able to exert up to 20 pounds of force occasionally and/or up to 10 pound of force frequently, or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.
