Salary Range:  $49,340.27 - $81,411.45 (Compensation negotiable within the salary range for fully qualified applicants. Salary rates are based on education, skill, and experience level as well as internal equity and market rates.)   • Providing services to disabled adults who are assisted level of care, wish to continue to live in the community and are in need of a social worker to assist with budgeting and making sure funds received are spent appropriately to meet their needs in order to live as safely and independently as possible.

RESPONSIBILITIES SPECIFIC TO SPECIAL ASSISTANCE IN-HOME (SAIH) SOCIAL WORK:
  • Assess the need of continual eligibility for the Special Assistance program
  • Develop, coordinate, revise, and monitor a service plan and economic worksheet with client, primary caregiver, family members and providers on a monthly basis or as needed
  • Coordinate and monitor of services needed monthly to assure health, safety and well-being
  • Monthly contact with providers, client and family members on service plan
  • Visits to client’s home on a quarterly and annual reviews and as needed in-between quarterly reviews
  • Extensive and thorough documentation
  • Assurance of compliance with Division of Aging and Adult Services for SAIH
  • Provide counseling to SAIH clients and families facing life crises regarding their physical and mental health, personal care, and living arrangements
  • Provide intake services for the Adult Services Unit half a day a week when on the intake rotation
  • Provide Information and Options Counseling to adults who are experiencing a major life transition and need assistance discovering and navigating services available to them using an in-depth and personalized approach
QUALIFICATIONS:
  • Master’s degree from an accredited school of social work; or
  • Bachelor's degree from an accredited school of social work and 1 year of social work experience; or
  • Bachelor’s degree in a human services field and 2 years of social work experience; or
  • Bachelor’s degree in a non-human services field and 3 years of social work experience
  • If the Social Worker position is a child welfare position:  Possession of a BSW with the Child Welfare Collaborative Scholarship/Waiver will replace one year of social work experience required.
  ADDITIONAL REQUIREMENTS: 
  • Valid Driver’s license
  • Previous experience working with older and disabled adults, long term care, medical terminology, and knowledge of community resources
  • Excellent organizational skills, ability to prioritize, multi-task and meet deadlines
  • Strong verbal and written communication skills to communicate effectively with clients and families, in-home community based providers, physician offices, and financial staff, as well as compete multiple plans, assessments, and reports
  • Capability to use sound social work judgment to mediate family conflicts and navigate non-traditional solutions in services
  • This is considered Light Work; lifting up to 20 pounds
