SOCIAL WORKER II Position # 674929                                                  Salary Range:       $46,311-$73,197 Purpose of Position:  The Care Management For At-Risk Children is a family centered Program that is responsive to the varying needs of children and their families. The Social Worker is to collaborate and coordinate with families to assure identification of and access to preventive, specialized and supportive services for themselves and their children. Clients served are age's birth to five years of age who meet program identified risk criteria. They may be at risk for developmental delay, chronic illness, toxic stress and social-emotional disorder or diagnosed with such problems. Clients are referred by nurses, doctors, or social workers where patient is in newborn intensive care nursery or normal newborn nursery, from client's primary care physician, health department staff, mental health staff, teachers, day care workers, Virtual Health/Data/Reports, Community Care Partners, DSS, Head Start teachers and self-referrals. The main goals of the program are to improve health outcomes and reduce costs for children enrolled in the program. Care Management for At-Risk Children (CMARC) assessment and evaluation forms will be used. Social Worker will complete initial assessments with input from parents. Social Worker will develop and implement a plan of care with parents/client's family. Documentation will be completed in the Virtual Health electronic system. Case records will be maintained, reviewed, and updated periodically. The Social Worker will encourage client to seek counseling/therapy as part of the care plan/needs assessment. Social Worker will adjust plan of care as client's needs change or as problems are resolved.  Social Worker will function as a community liaison for other county programs/services. The Social Worker may consult with other professionals providing care for client. Parents of client and Social Worker will be the base of the team. Other members of the team may include Public Health Nurses, Nutritionists, Day Care Teachers, and Social Workers from DSS, Mental Health staff, hospital employees, Children's Developmental Services Agency staff, Public School staff and other professionals working with children. The Social Worker collaborates closely with the local medical practice serving as the child's medical home to coordinate roles and responsibilities and ensure the child obtains necessary care. CMARC staff also work in close collaboration with their local Community Care of North Carolina (CCNC) networks to access care management histories, Medicaid claims and other vital records, and to coordinate care management services. CCNC networks also assist in quality improvement and in evaluating program effectiveness. The CMARC program social worker works with other disciplines within the health department's Care Coordination Unit to assure each child served by CMARC is linked to a specific medical home and CMARC Care Manager or program coordinator.  The CMARC Program Social Worker: 1. Assesses referrals, works to engage families and children in the program through face to face contact, advocates and collaborates with multidiscipline programs and providers of care to assure improved health outcomes according to a standardized plan individualized to the client's needs. 2. Develops service plans assuring that each child has a primary physician or medical home for ongoing service/care. 3. Supports and advocates for the social health of clients served by members of the Care Coordination Unit 4. Maintains a constant flow of information internally to the Maternal Health Coordinator and/or clinic nurses and the Care Coordination Unit Supervisor. 5. Participates as a member of the team providing support for all health department programs especially when covering for other care coordination staff positions/leave. 6. Approximate Percentage Breakdown of Position Activities: a.  50% Case Management, b.  20% Monitoring/Reviewing/Maintenance, c.  20% Counseling/Referral, d.  5% Program Development/Evaluation, e.  5% Resource Development Knowledge, Skills, and Abilities: Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group work and community problems. Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment. Knowledge of governmental and private organizations and community resources. Knowledge of laws, regulations and policies which govern the program. General knowledge of medical terminology, disease processes and their treatment is required in certain programs or settings. Skill in establishing rapport with a client and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client's problem. Ability to establish and maintain effective working relationships with administrative superior’s members of case load and their families, and with civics medical, social and religious organizations. Ability to train or instruct lower-level social workers, students, or interns. Ability to express Ideas clearly and concisely and to plan and execute work. Minimum Training and Experiment: Bachelor's degree in social work from an appropriately accredited institution; bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
