Social Worker III – Permanency Planning/Facilitator

The County is currently accepting applications for the position of Social Worker III (Permanency Planning/Facilitator). This position is primarily responsible for coordinating and facilitating Child and Family Team Meetings, Permanency Planning Reviews, and Transitional Living Plan/Transitional Support Team Meetings, as well as collecting and tracking associated data. This position will have additional duties as assigned to meet the overall needs of the Permanency Planning Unit and Social Work Services Division. Requires a thorough knowledge of social work principles, techniques, and practices and their application to complex casework, group work, and community problems; considerable knowledge of family and group dynamics and a range of intervention techniques; and skill in establishing rapport with a client and in applying techniques of assessing psycho-social, behavioral, and psychological aspects of the client’s problems. Requires a Master’s Degree in Social Work; or a Bachelor’s Degree in Social Work and one year of human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning; or a Bachelor’s Degree in a human services field and two years of directly related experience; or an equivalent combination of education and experience; requires proficient use of Microsoft Word and Excel, especially efficient word processing; experience in Foster Care and bilingual applicants preferred.
