Job Description
Primary Purpose
This position located in the Income Maintenance Division and is assigned to the Income Maintenance Division. The primary purpose of this position is to assist current Medicaid recipients, current Work First recipients and TANF Income level citizens to obtain transportation to access employment, job training, and or medical care as appropriate.
Essential Functions
95% Transportation Coordination
Manage transportation needs for clients who require assistance in getting to medical providers, to work and/or to employment activities.
- Assessment, arrangement, and coordination of transportation services for Medicaid, Work First and TANF eligible clients.
- Receives referrals, from Income Maintenance Caseworkers, Work First Employment Social Workers, Family Services Case Managers, or CPS workers of clients who are in need of medical transportation, those that have barriers to work, job preparation, or meeting their employment goal.
- Interviews clients in the office or by telephone who are seeking transportation services and to establish a plan.
- Determines eligibility for transportation services.
- Assists clients in assessing ability to meet own needs prior to providing any services.
- Chooses and arranges the most appropriate method of transportation for TANF clients
- Options are vouchers, written agreements, ridesharing, volunteers, the County Transportation System, or Contract providers. Determines eligibility of clients for assistance with car repairs, car insurance, and the appropriateness of requests for Medicaid transportation.
- Counsels clients in appropriate use of transportation resources and verifies services were used.
- Works closely with the county transportation system for coordination and reduction of no shows.
- Collects information and maintains files and reports as required for program review.
- Informs case managers of significant problems on cases which may require intervention. Any needed changes to client’s plan will be implemented. Provides ongoing communication to workers relative to client’s employment or medical transportation plan.
- Participates in team meetings and staffing of cases as appropriate.
- This worker will have client contact by phone or through the interview process to assess the client’s need for transportation.
