Apply online at https://carteret-apply.pdsvista.com The Job Description General Statement of Duties Performs intermediate advanced human support work determining social service needs and providing case management services to adults who are deemed incompetent, acting as surrogate decision maker on their behalf, determining available programs and services, developing service plans, planning and coordinating the provision of services, counseling clients, collaborating with community service providers to meet the needs of clients, preparing and maintaining files and records, and preparing reports. Distinguishing Features of the Class An employee in this class coordinates and arranges for the provision of service needs for adults who have been deemed incompetent. Work involves serving as the surrogate decision maker on their behalf and as the agent of the Human Services Director. Work also involves making referrals and coordinating with outside community agencies for the provision of services. Work is performed under the general direction of the Social Worker Supervisor III – Adult Services and is evaluated by a review of records, reports, discussions, client feedback, and by observation. ​Duties and Responsibilities/Essential Duties and Tasks
  • Provides case management to arrange and coordinate an array of services including appropriate housing, medical, crisis, mental health, financial, social functioning, family consultation, and court interactions to assist the ward in obtaining daily needs and remain safely in the least restrictive environment.
  • Acts as a surrogate decision maker on behalf of wards deemed incompetent and as an agent for the Human Services Director.
  • Makes referrals to other community services or agency services.
  • Prepares reports and other types of correspondence; maintains case logs; prepares and maintains files and records.
  • Completes quarterly, annual, and six month reviews and provides status reports to the court which include evaluating the ward’s potential for restoration, modification, or transfer of guardianship.
  • Enters a variety of data into computer system.
  • Testifies in court.
  • Maintains a close working relationship and acts as a liaison with mental health, facility administrators/managers, clerks of court, law enforcement, medical providers, and other agencies that serve adults.
  • Serves as back-up to Adult Protective Services in an on-call basis as needed

Physical Requirements
  • Work requires the occasional exertion of up to 50 pounds of force to move objects.
  • Work frequently requires sitting, standing, walking, using hands to finger, handle, or feel, speaking or hearing and repetitive motions, occasionally requires climbing or balancing, stooping, kneeling, crouching, or crawling, reaching with hands and arms, tasting or smelling, pushing or pulling, and lifting.

  • Vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly, or quickly.

  • Work requires preparing and analyzing written or computer data, operating machines, operating motor vehicles or equipment, and observing general surroundings and activities.

Desirable Education and Experience Graduation from a four-year college or university with a degree in human services, social work, or related field and moderate experience in human services and counseling; or an equivalent combination of education and experience. Special Requirements Complete Adult Protective Services Modules I & II and Guardianship training within one year of hire. Valid North Carolina driver’s license.
