Human Services Coordinator – II

Primary Purpose of Organizational Unit: The Meals on Wheels program provides one hot, nutritional meat to homebound, elderly and/or disabled adults daily. Meals are delivered by volunteers who donate their time to provide both socialization and nutritional/ health support services,

1. DIFFICULTYThis position will independently perform agency standardized assessments to identify eligibility of citizens for a home-delivered meal and must understand and adhere to all applicable local and state policies including, but not limited to, the Division of Aging and Adult Services program policy. This position should also have a general knowledge of Food Services policy and laws. In addition, this employee will serve as an interagency and multidisciplinary team member and may refer clients to a variety of services to meet financial psychological, emotional, or other needs. Intricacy - This position works closely with homebound, elderly, and/or disabled clients who have a broad range of service needs. This position also works with a volunteer base of over 200 whose age, education, backgrounds and experiences vary greatly. This position must be able to use analysis and judgment in performing client assessments and determining other referrals or resources that may be appropriate to provide the best quality of life to the clients This position must have excellent oral and written communication skills and will interact closely with other agency partners and the community in both outreach and fundraising activities.
2. Work Schedule:

The Meals on Wheels Program Human Services Coordinator's work hours are from 8:00 a.m. through 5:00 p.m. weekdays with a one-hour lunch break. A deviation from the standard work schedule may occur occasionally with outreach or community activities that are scheduled in evenings or on weekend as approved by supervisor.
